I voluntarily submit this entry to compete in the Texas Cotton Belt Karate Nationals and assume full responsibility for any and all damages, injuries, and losses I or anyone accompanying me may sustain or incur while attending or participating in the tournament.  I waive any claims against the promoters, operators or sponsors, Sports complex, individually or otherwise.  I also grant the Texas Sport Karate Insider and promoters of the Texas Cotton Belt Karate Nationals permission to use any pictures taken of me at said tournament.
